The peroneus longus muscle arises from the lateral condyle of the tibia and the proximal fibula, and it inserts on the plantar surface of the medial cuneiform bone and the proximal first metatarsal bone. The peroneus brevis muscle originates more distally, from the distal two-thirds of the lateral fibular shaft, and inserts on the lateral aspect of the base of the fifth metatarsal bone

Transverse images are obtained at the supramalleolar, malleolar, and inframalleolar segments while tilting the transducer perpendicular to the tendons.
